This is a case of toric IOL. Preop- a case of mature cataract with astigmatism of 4.24 D , with steep axis at 64 degrees. A toric IOL was planned, with 20D ,5.5 cylinder power. Surgery was uneventful. Howewer, post surgery patient had 9D astigmatism.
To asses this refractive surprise, i went to www.assort.com website . While entering all the data, i realized that the lens was at 160 axis and it has to be rotated to 65 degreesI. I realised that it was amnnual error while reading the chart and placed lens at 160, instead at 60 degrees. so, a rerotation of IOL was planned at 3weeks. After marking the axis, anterior and posterior capsule were separated. then the IOL was slowly rotated to desired axis. Post operatively, the cylinder was only 0.5. Hence in case of refractive toric surprise, it is possible to use these websites to plan the future action.
