This video demonstrates the management of Posterior capsular tear during Cataract surgery under topical anesthesia.The pearls & pitfalls fo PC Tear management and anterior vitrectomy are highlighted.
A PC tear is noted during Irrigation & aspiration.
1. Without removing the Irrigation Handpiece, Dispersive OVD is injected into the anterior chamber to prevent Collapsing of the Anterior Chamber.
2.The irrigation is handpiece is then removed and OVD is further injected in to the AC.
3. Using A Dr Haldipurkar Capsulorhexis forceps, A posterior capsulorhexis is created which makes the irregularly torn PC in to a well defined opening with strong continuous margins.
4. A multi piece IOL is placed in the Sulcus with optic Capture.
5. At this stage Vitreous disturbance is noted and triamcinalone assisted bimanual anterior vitrectomy is done.
Directions for Triamcinalone dilution for intracameral usage during anterior vitrectomy.
0.5 ml of Aurocort(Triamcinalone Acetate Aurolabs) is mixed with 1 ml of BSS in a 2 ml syringe . This is then used for staining & identifying prolapsed Vitreous.

Post operatively the patient did well with excellent visual outcome.
It is important to follow these patients for long with periodic dilated retinal evaluation .
