Posterior capsular rupture (Rent): How to insert IOL in the bag- thinking process

In this video, i will be talking about how to put the lens in the bag in case of posterior capsular rupture. It may be important in cases of premium IOL like multifocal and toric. First thing to check is whether the anterior hyaloid face is intact or not. If there is vitreous prolapse, we have to do anterior vitrecotomy first. IOL require support at haptic and optic haptic junction for stability. So for central vertical rent , we can placethe lens horizontally and if the rent is central and horizontal, we can place the lens vertically. In case of small peripheral rent, we can place the lens in the bag with haptics in perpendicular direction. In case of large peripheral rent, we can place the 3 piece lens in sulaus with optic capture. Plate haptic IOL is difficult to put in the bag because of the stretch on the bag. In case we want to do PCCC, ensure to keep the posterior hyaloid concave with visco. always check for vitreous, if present do anterior vitrectomy. Check for vitreous strands going to incisions. take precautions not to enlarge the rent. i enlarge the incision to avoid any resistance to placing lens. Put th elens in the anterior chamber first and then very gently using sinskey or kugllen hook place the lens in the bag. Do passive visco wash. Confirm there is no vitreous before closing the case.
