Phacoemulsification in Peters anomaly, microcornea

In this case i am tackling a case of peters anamoly, with microcornea, central corneal opacity and adherant lens. Because it was a microcornea, i made sclerocorneal main incision and posterior limbal side ports. After staining, i used hyluronate to protect the endothelium. I used a vannnas scissors to cut the adhesion between the lens and the cornea. Make a large CCC so that the phaco is easier and the lens can be placed the bag. It is a soft catarct. i used a carausel technique minimising the phaco power. I used haigis formula for lens calculation. lens power was 35. i used a thicker cartidge for thick lens. For corneas above 9mm , we can use normal lens. howewer, for smaller corneas , cusom made lens may be required. visco removal should be meticulous in this case to avoid IOP spike.
