Generalized zonular weakness secondary to pseudoexfoliation is a relatively common co existing pathology in elderly patients undergoing cataract surgery . In this Video practical tips are demonstrated on how to minimise complications during nucleus management in this complex case.
Unintended accidental intracapsular extraction is the most common associated complication owing to the generalized zonular weakness and a large bulky nucleus.
3 important principles will minimize this complication.
1. Good hydrodissection to ensure that the nucleus-Cortex complex is totally free from its attachment with the loose capsular bag before attempting to prolapse the nucleus out of the bag.
2. Early insertion of the CTR . just after hydrodissection will provide equatorial stretch and support to the bag to withstand stress during nuclear manipulation
3. Bimanual technique of wheeling out the nucleus is a critical technique to master during prolapsing the nucleus out of the bag especially in such eyes with loose zonules.
